notices -
These
notices are sent out the 15th of the month after you are
2-˝ months in arrears with a $5.00 late charge; on the 1st
day of the 4th
month you
will receive a $50.00 late fee.

month or
after the 20th of the month, this all depends if your
contractor gets their reports in
early and/or on time.
Longevity Awards - Will be Friday May 30th 2008, starts at 6:30 pm
Effective January 1, 2008,
the administration of the dental benefits of the Pipe Health and Welfare Fund of
Colorado will be handled by Fringe Benefit Services, Inc., instead of Cigna.
This means that itemized statements for dental services submitted after
2007 should be sent to:

This change in administration is
being made to provide more timely payment of the $400 combined family calendar
year maximum. Also, to ensure there
is no delay. Please make sure that you provide the following when you send
dental claims:

Member’s name
2.
Member’s SSN
3.
Member’s Address
4.
Copy of Dental bill
5.
Copy of receipt showing payment
6.
Write “Colorado Pipe Dental Benefits”
on the
bill or receipt
