DENTAL CLAIMS

As of January 1, 2008, the $400 per family, per year reimbursement will not come from Cigna, they will be handled by Fringe Benefit Services, Inc.

This change in administration is being made to provide more timely payment of the $400 combined family calendar year maximum. This reimbursement is only for active members working in the field and have an hour bank or paying Cobra. Also, to ensure there is no delay Please make sure that you provide the following when you send dental claims:

Member’s name, SSN, and address
Copy of Dental bill
Copy of receipt showing payment
Write “Colorado Pipe Dental Benefits” on the bill or receipt
